Pediatric Therapy Billing and Payments

As a courtesy to our families, UCP of Central Arizona verifies insurance benefits prior to treatment, obtains authorization if necessary, and bills a patient’s insurance plan. Below is our complete list of our current contracts.

Planes de seguros comerciales

UCP accepts all major private insurance plans. Coverage for therapy services may vary from plan to plan. Therapy services may not be covered by a commercial insurance plan or may be limited. Often, coverage may vary slightly from the quote provided by your commercial plan. We will do our best to help, but the family must know their individual coverage. We encourage each family to check the benefits of their plan with their insurance company.

UCP will provide services to a child with the understanding that if any or all of the services are not covered by insurance, families will accept financial responsibility for the services rendered.

Arizona AHCCCS Plans                                                     

UCP accepts multiple AHCCCS plans. If your child has an AHCCCS plan, we will verify eligibility and benefits and discuss therapy services coverage with you.

Private Pay:

Families have the option of paying privately for therapy services. Below are rates for standard of care services.

Evaluaciones de terapia: $250

Cancellation Policy: If a child is scheduled and cannot attend the session, we require 24-hour notice. If the session is a no show or not cancelled within 24-hours, it may result in a late cancellation fee.
